I authorize American Care Team to conduct, or hire services to conduct a background investigation of me. I authorize any parties contacted to release information to my employer or its agent (e.g., a consumer reporting agency) regarding my previous employment, criminal history on the federal, state, and local levels, military records, credit history, driving record, drug test results, academic records, licenses and certifications, and any other information. I release all persons and entities from liability for damages that may arise from the release of this information.
I waive all provisions of the law prohibiting the disclosure of information. I understand that American Care Team and its agents cannot guarantee the accuracy of any information reported to it by third parties, and I release American Care Team and its agents from liability for damages that arise from errors or omissions in my background investigation.
Submission authorizes and approves that you have fully understood the Standards and Procedures of this form. If you have any questions, please contact the Office Manager at American Care Team before submission since failure to follow these rules could result in disciplinary action up to or including termination. All probationary periods are a minimum of 90 Days for all American Care Team staff.
American Care Team, LLC is licensed as a residential service agency by the Maryland Department of Health and Mental Hygiene, Office of Healthcare Quality. We are authorized to provide care in the State of Maryland. License #R3891R.
1. Do not document that you were caring for a client and you were not onsite or doing the work.
2. You must document and make a note of the time of your visit or take videos or post on any social media about your job or the location of your patient for any reason.
3. When you are documenting on your notes, do not put any false information. You must document only work that YOU have done.
4. You must not do anything fraudulent or against any City, County, State or Federal Laws
5. You must not call off of work at the time it is scheduled for you to report onsite to any patient. This can result in termination.
6. You must clock in and clock out every single visit to your patient. If you fail to clock in or out and do not complete a timesheet. You will not be paid until you are able to verify you have completed work onsite at the times you have confirmed.
7. The State of Maryland or American Care Team systems will tell us if your times are conflicting with another time and flag you. You cannot be in two client's homes at the same time, there must be traveling time. You must give the Office Administrator your schedule asap, so the system can be updated. You must email care@americancareteam immediately of any changes.
8. You must immediately notify the Office Manager at care@americancareteam or via text at 240-581-2918 if you have any issues with clock-in or clock-out or you will not be paid.
9. You are there to care and support your patient at all times. There must (3) ADLs (bathing, grooming, toileting, ambulating, feeding, etc.). You are not allowed to make or receive any personal calls for any reason while you are working in a patient’s house unless you are on an approved break or meal break which must be approved in writing or directly from the Office Manager in writing. Your meal break cannot extend longer that 30 minutes for any reason.
10. Do not give any medications to any patient for any reason unless you are licensed, trained or authorized to do so by the Director of Nursing or the patient’s Physician with specific directions in writing.
11. You are not to transport clients at any time in your personal vehicle without permission.
12. Do not accept any gifts from any patients or ask for any gifts or payments or any money.
13. Do not accept, borrow or ask for any money from the client or family member.
14. Do not ask or attempt to use any patient or company issued debit card, food stamps (EBT) or other financially owned information (checking accounts, etc) for any reason.
15. You must attend all training in-person onsite or through remote video conference calls as required or necessary by American Care Team. Failure to attend mandatory training or complete mandatory documentation for American Care Team HR for State of Maryland compliance will immediately pause your paycheck or payroll until you have become compliant.
16. You are not permitted to post any information, videos, comments, photos or any data (e.g. positive, negative or other information) on any social media platforms (e.g. YouTube, Instagram, TikTok, SnapChat, Twitter, BlueSky, Facebook or similar) for any reason without written permission from the American Care Team Director of Nursing or Chief Technology Officer.
17. You are not permitted to speak to the general public or any State of Maryland, County or Federal officials on behalf of American Care Team at any time and for any reason. Please direct all media or inquiries to

18. Any theft, violence, threats, intimidation or other criminal actions will result in immediate termination and/or referral to a local jurisdiction law enforcement agency.
